Maryland runs its own marketplace, Maryland Health Connection, rather than routing residents through HealthCare.gov. Open enrollment for 2027 coverage runs November 1, 2026 through January 15, 2027, with a December 15 deadline for coverage starting January 1.
Maryland has among the highest household incomes in the country, which creates a specific problem after the ACA’s enhanced premium tax credits expired at the end of 2025: higher-earning Maryland households were disproportionately helped by those enhanced credits, because the old subsidy structure cut off eligibility entirely above 400% of the federal poverty level. With enhanced credits gone, many Maryland families who were receiving meaningful assistance in 2025 now receive none — and are seeing the full unsubsidized premium for the first time.
That makes a private PPO comparison genuinely worth running in Maryland this year. When marketplace subsidies drop out, private health-based plans frequently price better for healthy applicants than the unsubsidized marketplace rate, since they’re rated on health and age rather than being community-rated.
